چکیده

Editor—In their response 1 to my criticisms 2 of their paper 3 Sell and colleagues accept some of my points, and refute others. On most of the latter we will simply have to agree to differ, but, not surprisingly , I cannot accept their quoting my own review 4 in support of their belief that the solutions which they used were isobaric! Their quote from the article ('Given the normal variation [of CSF], it is necessary that solutions that are to be predictably hypobaric or hyperbaric in all patients have baricities below 0.9990 or above 1.0010 respectively') is correct, but it is taken out of context, and ignores the key relevance of the words 'given the variability.. .' and 'predictably', to say nothing of the arguments that follow in the rest of that paragraph. These lead to the specific conclusion that the word 'plain' is a more appropriate descriptor so I would ask them to consider the paragraph in full, not just a selective quote.

Minimum effective local anaesthetic dose of isobaric levobupivacaine and ropivacaine administered via a spinal catheter for hip replacement surgery.

BACKGROUND Continuous spinal anaesthesia with spinal catheters allows incremental dosing of local anaesthetic and, consequently, less haemodynamic changes. However, little is known about the required doses. Therefore, we designed a study to assess the minimum effective local anaesthetic dose (MLAD) of levobupivacaine and ropivacaine in this context. METHODS Forty-one patients undergoing hip r...

Is Spinal Anaesthesia Useful in Day Surgery?

58 Vol. 10 No. 2, April-June 2008 Introduction: An increasing number of day-case surgical patients are challenging the presently used methods of anaesthesia:. Surgical anaesthesia should be fast, reliable with rapid recovery and minimal side effects. To compete with modern ambulatory general anaesthesia, knowledge of special spinal anaesthesia techniques is essential. 'Walk-in, walk-out' spinal...

Influence of the baricity of a local anaesthetic agent on sedation with propofol during spinal anaesthesia.

BACKGROUND This study examined the effect of different levels of spinal anaesthesia, induced by solutions of different baricity but containing the same amount of local anaesthetic agent, on the requirement for sedation with propofol. METHODS Thirty-six patients undergoing varicose vein surgery under spinal anaesthesia were randomly allocated to receive tetracaine 15 mg in 3 ml of either gluco...

Low-dose spinal versus epidural anaesthesia for delivery and expected caesarean section

Regional anaesthesia is associated with significantlly lower mortality among obstetric patients, but the optimal technique for delivery and cesarean section remains to be determined. Conventional epidural analgesia has disadvantage of slow onset and higher rate of instrumental delivery while spinal anaesthesia in standard doses causes hypotension and bradycardia which might further compromise c...

Efficacy and Appropriate Dosage of Isobaric Ropivacaine for Spinal Anesthesia in Patients Undergoing Elective Lower Limb Orthopaedic Surgeries

Background: The dose response of ropivacaine has not been extensively determined yet. This study was conducted to estimate minimum effective local anaesthetic dose and to assess the duration of sensory and motor block, and side effects if any, of intrathecal administration of ropivacaine for lower limb surgeries. Materials and Methods: 120 patients aged between 20 years and 60 years of either s...

Sensitivity, specificity and predictive value of the sensation of warmth as a method of detecting inadvertent subarachnoid injection of local anaesthetic when performing extradural blocks.

In order to test if the rate of onset of sensation of warmth in the legs after the injection of 0.5% bupivacaine might discriminate between subarachnoid and extradural injection, 150 urological patients were allocated randomly to receive either spinal anaesthesia with isobaric (IS) or hyperbaric (HS) 0.5% bupivacaine, or extradural anaesthesia with isobaric 0.5% bupivacaine. The volume of the l...
